Unofficial Forms and Certificates

A number of forms/certificates produced by Scouters. Currently we have a number of health/camp forms, and some activity authorisation certificates.

These files are all in Rich Text format (RTF), and can be opened in a number of word processing packages, e.g. Microsoft Word.

Activity Certificates

Axe & Knife Certificate

This is a small Certificate Booklet (similar in size to the Scout Record Pocket Book), which can be used as an authorisation scheme (in association with a suitable training and test scheme) to clear Scouts for the use of Axes and Knives at Scout Activities.

Lamp & Stove Certificate

This is a small Certificate Booklet (similar in size to the Scout Record Pocket Book), which can be used as an authorisation scheme (in association with a suitable training and test scheme) to clear Scouts for the use of Lamps and Stoves at Scout Activities.

Camp/Holiday Information Sheet

This form is the result of a long succession of similar forms which developed over a period of some 10 or 15 years. It brings together in one place all the information that any Parent/Guardian would need to know about any Camp/Holiday you may be organising.

Kit List Sheet

This form is the result of a long succession of similar forms which developed over a period of some 10 or 15 years. It is a distillation of many years experience of organising Cub Scout and Scout Camps.

Camp/Holiday Health Information Form

This form is the result of a long succession of similar forms which developed over a period of some 10 or 15 years. It brings together in one place all the information that will (hopefully never) be required by a local Accident and Emergency unit or Local General Practicioner, should an accident occur to a young person in your care. This particular form came into being in it's present form at the request of my District's Group Scout Leaders meeting, and with input from the Local A & E unit. It is worded in a way that is easily understood by non-medical people.

Camp/Holiday Health Information Form

(Camp Site Service Crew)

This is a variant of the 'Camp/Holiday Health Information Form', and came about at the request of an Assistant Camp Site Warden. On this particular site the Service Crew had quite a few older Scouts/Explorers for whom he needed a Health Form when they stayed on site for weekends etc. The main variation being the period of time for which the form is in force.
