It brings together in one place all the information that will (hopefully never) be required by a local Accident and Emergency unit or Local General Practicioner, should an accident occur to a young person in your care. This particular form came into being in it's present form at the request of my District's Group Scout Leaders meeting, and with input from the Local A & E unit. It is worded in a way that is easily understood by non-medical people.

This is a variant of the 'Camp/Holiday Health Information Form', and came about at the request of an Assistant Camp Site Warden. On this particular site the Service Crew had quite a few older Scouts/Explorers for whom he needed a Health Form when they stayed on site for weekends etc. The main variation being the period of time for which the form is in force.
